Use call forwarding on your iPhone, to your truphone number. Then all calls will be routed VOIP. Same way with skype, when they get push.

As an aside, Nimbuzz told me that they are also working on their new client for iPhone’s new OS3.0. Just like Truphone, it is going to work with the new options OS3.0 is offering for VOIP solutions. Unlike Truphone, they work with all your regular IM clients, and they include skype and other voip providers. I loved it on my ipod touch and the new version will be one of the first installs as soon as I get my new 3GS delivered.
